Here at Safety Connection, we recently came across this mailing from the National Association of Tower Erectors:

Dear NATE Members:
Over the past two weeks, the tower erection, service and maintenance industry has experienced the loss of four tower climbers through four separate accidents. NATE is asking all members to take the time today to talk to your crews about safety.
Please remind your climbers of the importance of following the training they have received. Emphasize that 100% tie-off is mandatory. Above all else, take this opportunity to let them know that their decisions are essential; their lives depend on the choices that they make.
Two months ago NATE members pledged their commitment to safety by filling a wall with their signatures at the NATE Annual Conference & Exposition. That wall was a symbol of our association's dedication to safety. NATE is asking you to again demonstrate your leadership by delivering a message to your employees to remain vigilant and dedicated to safe operating practices at all times. Together, NATE members will continue to make a difference.
